Community Health Needs Assessment 2016

Community Health Needs Assessment 2016

We provide specialized rehabilitative services to our patients in the communities we serve. We often see patients who are recovering from disabilities caused by injuries, illnesses, or chronic medical conditions.

Community Health Needs Assessment 2016

In 2016, Summa Health, a joint venture partner-owner of Western Reserve Hospital, collaborated with Akron Children’s Hospital and Cleveland Clinic Akron General to conduct a Community Health Needs Assessment (CHNA). During the CHNA process, epidemiologic data were reviewed and input was obtained from community leaders and community residents. All of this information was used to develop a list of significant health needs for our community.

Each collaborating partner has issued a separate CHNA report specific to each System’s hospital facilities covered by the collaborative analysis. The purpose of this CHNA is to improve the health of our community. The information presented in this report will also act as a resource for other community groups working toward improving the health of the community. In addition, this report will fulfill the CHNA requirements established by the Patient Protection and Affordable Care Act for the hospital facilities listed.
